Wagner Professionals plan home or hospital care. With one phone call, an intake coordinator determines how to best meet the needs of the client and family. From there, a nurse provides an on-site evaluation and suggests a plan of coordinated care consistent with all involved medical professionals, and according to the personal needs of the individual.

Registered Nurse: RNs assist patients in reaching an optimal level of health based on the client's physical, mental, and emotional state and the physician's prescribed plan of care. The RN assesses the client needs, provides direct hands-on care based on these needs, and reports the outcome of the prescribed treatments and the patient's response to the overall plan of care to the attending doctors. The nurse also coordinates with other professionals involved, such as Physical Therapists and recommends care that may not be in place. All Wagner nurses are board certified and NY licensed.

Licensed Professional Nurse: LPNs are brought in to cases that do not require the level of care practiced by a RN. Very often, as a client improves or stabilizes, the RN will continue to monitor the client's status, but turn over the daily care assessments to a LPN.

Home Health Aides: Aides perform a wide range of patient care as defined and supervised by an RN. This includes activities of daily living such as bathing and morning care, assisting the patient with mobility and self administered medications, meal preparation, and prescribed exercises as determined by a Physical Therapist.

Wagner also provides Personal Care Aides for assistance with personal care, companionship, light laundry, shopping, and housekeeping.
